Services
The program offers a network of community based residences or respite within family and intermediate resources under contract with the CISSS of Laval, Jewish Rehabilitation Hospital. These residences vary in size but the majority are smaller residences that can accommodate between 3 and 20 people. The owner lives there or there are staff to provide services to the residents.
A team of professionals follow the residents and support their social integration.
This program also has the mandate to manage respite care for children with physical disabilities.
Clients
Persons with a physical disability who are less than 70 years of age who are unable to live alone in the community despite the assistance of the home care team of the CISSS of Laval. The person must not require ongoing intensive nursing care.
Location of Intervention
The clinicians provide services in the residences or through activities in the community.
