After you leave the hospital, Perinatal Services at the Centre intégré de santé et de services sociaux (CISSS) de Laval will receive a continuity of care form with information about you (the mother) and your baby.
This form will indicate the type of follow-up you need based on your medical needs or how your hospital stay went. The goal of post-natal checkups is to:
- Help new parents keep their babies safe and healthy
- Detect potential problems and refer you to the appropriate resources
- Provide breastfeeding support
- Help you welcome and adapt to the new member of your family
- Help dad get involved
Telephone assessment
The CLSC will contact you within 72 hours of your discharge from the hospital. If you receive a call with the indication of private call, it is very likely that the CLSC is trying to join you.
If you have not received a call from us, you can contact us for the postnatal follow-up at 514 262-5814.
During the call, the nurse will:
- Assess mom and baby’s physical well-being
- Support breastfeeding
- Ensure your family is adjusting to being back home
- Refer your family to the right resources
Post-natal checkups with the nurse at the CLSC
This follow-up includes:
- A physical exam of mom and baby
- An evaluation of your newborn’s behaviour, diet, and stool and urine patterns
- A continuation of the education started at the hospital, such as breastfeeding support
After the evaluation, the nurse may decide that you need more help. If so, your appointments will be at the CLSC clinic. If a problem is found (which the nurse will assess according to criteria set by the CISSS de Laval), the nurse will refer you to the right resource.
Breastfeeding drop-in centres
You can go to one of Laval’s four breastfeeding drop-in centres without an appointment. You’ll get information on different breastfeeding topics and can talk with other breastfeeding mothers. For more details, consult the brochure that the nurse gave you during your home visit.

Vaccinations for your baby
You should make an appointment for your baby’s first vaccinations, as immunization is the best way to prevent harmful diseases.

Info-Santé is a phone service that lets you talk with a nurse about a health problem. You can call from anywhere in Quebec, 24 hours a day, 7 days a week, by calling 811. After assessing your health problem, the Info-Santé nurse may advise you on how to treat the problem or refer you to the best resource in the public health network for your situation.
