The teams in this program provide tailored services to clients of all ages who have the following two characteristics:
- Has a diagnosis of a physical or intellectual disability or autism spectrum disorder (e.g., Asperger syndrome).
- Meets the definition of someone with a disability, i.e., the disability causes significant and persistent impairments.
Staff on these teams provide professional services and at-home assistance. They can assess the needs of clients and refer them to specialized or community services, if need be. After the multidisciplinary team makes an overall assessment, different services can be provided based on the situations of disability that the person experiences.
