    The goal of this program is to provide financial assistance for travel for anyone with an intellectual, psychiatric, organic mental, or physical (motor, visual, auditory, or speech-language) disability. The subsidy covers travel to institutions in the network of the Ministère de la Santé et des Services sociaux (the most appropriate and closest) that provide adaptation, rehabilitation, diagnosis or treatment services for the impairments indicated in the person’s intervention plan.

    A medical diagnosis is required to be eligible for the program.

    Eligible trips

    Costs incurred for the trips identified below will be reimbursed to the disabled person or his sponsor, according to the administrative rules defined.

    • Any trip greater than 50 km (round trip) between the place of residence and the most appropriate establishment of the health and social service network offering adaptation / rehabilitation, diagnostic or treatment services related to impairments recognized by the RAMQ and planned in the intervention plan.
    • Trips of less than 50 km (round trip) may be eligible in the following cases:
      • for preschool children (0-5 years) attending an early stimulation center;
      • for repetitive treatments such as hemodialysis;
      • for medical treatment and adaptation-rehabilitation services that require intensive follow-up.

       

    Definitions to better seize the scope of the above criteria

    Structured program with adaptation and rehabilitation objectives and which is specified in an intervention plan.

    Refers to any treatment, recognized by the RAMQ, essential and required almost permanently (for life) to maintain the state of health and the level of autonomy of the disabled person (eg: hemodialysis).

    Refers to any medical treatment or any adaptation or rehabilitation service recognized by the RAMQ, required for a determined period in order to improve the state of health and the level of autonomy of the disabled person.

    The minimum frequency of treatments or services is once a week or every two weeks for a period of 3 to 6 consecutive months.

    The minimum total number to be eligible for financial assistance is therefore 12 trips to the same establishment.

    Place where early stimulation services are provided in establishments of the health and social service network or by non-network resources when the programs are recognized by the network, with the professional support of a network establishment.

    Important note

    • Any travel must be recognized in advance as clinically relevant by the person responsible for the file completing the grant application.
    • These services must be provided by a professional from the Quebec health and social services network, member of the professional order corresponding to the profession, in a nearby establishment recognized by the Health and Social Services Network.
    • The financial assistance provided for by the program does not apply to travel by users to private establishments (eg: private practice, private clinic, etc.

     

     

     

    Roles and responsibilities

    CISSS staff member

    • Performs a yearly needs assessment to renew the subsidy. 
    • Creates an intervention plan that indicates the number of visits and the institution.

    Users and their families

    • Submit the visit attestation form to the health professional who provides the service.
    • Keep parking receipts for the visits listed in the intervention plan. 
    • Return the duly completed visit attestation form and parking receipts to the CISSS de Laval within the deadlines indicated in the acceptance letter.

    CISSS

    • Confirms in writing that the subsidy has been granted for the current year.
    • Pays the subsidy as per the agreement's terms and conditions.
